Upward Bound at Lincoln University On-Line Application

Upward Bound is a federally-funded TRIO program that provides academic support to low-income and/or first-generation college bound students. To qualify, students need to either meet TRIO low-income guidelines or a first-generation college student meaning neither parent has obtained a bachelor's degree. Students must also, attend one of the target high schools listed below.

Guidelines For Eligibility

  1. Be a U.S. Citizen/Permanent Resident
  2. Be in the 9th or 10th Grade
  3. Current student at a Lincoln University Upward Bound Program target high school:

Release Of Records and Parental Permission Form

As a parent/guardian of the above-named student, I am signing this blanket permission form for the entire time my son/daughter is enrolled in the Lincoln University Upward Bound Program. To authorize the following:

 

  1. The right to request my son's /daughter's transcript from the above High School to release all school records, or copies thereof, of the above named student including transcripts, health records, psychological and social reports of a pertinent nature. Information received shall remain in strict confidence and be used by administrative staff only.
